Documents Needed to Receive Services

We understand that asking for help can feel overwhelming. Our goal is to make the process as smooth as possible. Please review the required documents below and bring all applicable items with you so we can better assist you and your household. Having these items ready will help our team process your application and determine eligibility as quickly as possible.

Copy of a photo ID for all household members
Copy of birth certificates for all household members
Copy of Social Security cards for all household members
Copy of the lease agreement for the rental unit the participant will be moving into
Three months of paycheck stubs
3 months of bank statements (checking and savings)

Documentation of any additional income being received
Verification of current employment or award letter

EVICTION ASSISTANCE

Please also include:

3-day notice (if applicable)
Verification of eviction
Completed landlord verification form
Executed lease agreement
Landlord deposit agreement form

* If you are experiencing homelessness, please register with the Homeless Task Force (954-563-HELP) prior to your appointment in order to obtain a Service Point Number.

* Funding is based on a first-come, first-served basis.

*All documents MUST be complete and accurate to avoid delays in services.
